What Fields Are Available in the Form Builder in Gearbelt?

Learn which fields are available for building form templates in Gearbelt, and how each one works

The Form Builder in Gearbelt allows you to build custom templates for inspections, schedule forms, and repair forms. You can tailor forms to match your team’s workflow and update them as your processes change.

To access the Form Builder, go to:

  • Inspections

  • Schedule Forms

  • Repair Forms

Field Configuration

All fields include a configuration menu where you can:

  • Change the field label

  • Mark the field as required

  • Adjust field-specific settings

Below is the complete list of available fields across all form types:


Unit Fields

Unit Reading

Allows you to capture asset readings.

Configuration options:

  • Select unit type (Hours, Kilometers, Miles)

  • Allow lower readings (if required)


Text Fields

  • Single Line Text
    Allows a single line of written input. Supports hyperlinks.

  • Multi Line Text
    Allows written input across multiple lines for longer responses.

  • Prefilled Text
    Displays preset written content that cannot be edited by form users.

    Additional options:

    • Edit text size

    • Change text style

    • Adjust text colour

    • Add photos

    • Add links


Date (Timezone Aware) Fields

  • Single Date Time
    Allows manual date and time selection (timezone aware).

  • Single Date
    Allows manual date selection (timezone aware).


List Fields

  • Dropdown
    Allows selection from a source list.

    Source list can be:

    • Manual typed

    • Link to a list

  • Location

    Allows selection from your Locations list.


Table Field

  • Default Table
    Allows flexible row entries during form completion.

    You can:

    • Add columns

    • Choose column types:

      • Single Line Text

      • Number

      • Single date

      • Single time

      • Dropdown: Static/List


Pass / Fail Fields

  • Pass/Fail
    Allows users to record a pass or fail result.

    Configuration options:

    • Change the label

    • Modify the question text

    • Configure the display text

    • If marked as Fail, a Create Issue button appears.

  • Pass/Fail/NA

    • Includes the same configuration options as above.

    • Allows users to record pass, fail, or not applicable.

    • If marked as Fail, a Create Issue button appears.


Signature Fields

  • Sign Manually

    Allow users to record the signature manually, can be used by anyone without a Gearbelt or Sitemate profile.

  • Signature

    Allows users to to record the signature associated with their profile.


Photo Field

  • Photos
    Allows users to upload images during form completion.
    Photos can also be added while editing the template.
