How to Report Incidents and Near Misses via the Sitemate App on Dashpivot

Single Submission Sitemate Links let anyone submit a form via the free Sitemate app without needing a Dashpivot account. This works well for incident reporting, enabling anyone on site to report an incident or near miss electronically and submit it directly into your Dashpivot account. All submitted reports are automatically organized into a register in Dashpivot, making them easy to search and filter when needed.

If your incident report template is set up as a workflow, you can use the workflow notifications feature to notify a Dashpivot user (e.g. your Safety Manager) as soon as a report is submitted.

📓 Please note: none of the people submitting incident reports need to be paid Dashpivot users — they use the free Sitemate app to submit reports at no cost.

How to create a Single Submission Sitemate Link for incident and near miss reports

  1. Go to the team folder where you want your reports to be submitted

  2. Go to the Templates section

  3. Go to the Links tab

  4. Make sure you are in the Single Submission section (this is the default). If you are in the Ongoing Submission section and create a link, every person who submits a report will become a paid Sitemate Form User

  5. Click Create Link

  6. Give your link a name and select your report template

  7. Click Create

Note: if you don't have an incident or near miss report template yet, you can access several free options from the Public Template Library.

Once created, distribute the Sitemate Link to the relevant personnel, or put up QR posters around your site so anyone can submit a report when needed.

How to get workers to submit incident reports

There are two ways to distribute your newly created link:

  • Copy the link and send it as a URL — for example, via email or text message

  • Download a QR code poster that opens the link when scanned with a smartphone, and stick it on the wall of your site or office

The QR poster works particularly well for incident reporting — anyone can scan the nearest poster and submit a report straight away, without needing to be sent a link directly.
