Form headers in Dashpivot include standard information for branding and document control. Some header elements can be modified (like your logo and brand settings), while other metadata is fixed to support traceability when forms are downloaded or printed.

Prerequisites

  • Access to the relevant Dashpivot workspace.

  • Permission to access Home Folder > Settings.

  • Permission to edit templates (to update the Template ID).

  • An image file available if you want to upload a logo or header image.

Step-by-Step Instructions

  1. Review what appears in your form header.
Open any form and look at the header area.

  2. Update the workspace logo (shown in the header).

    1. Go to Home Folder > Settings > Branding.

    2. Upload or replace your company logo.
The updated logo will appear in the header on forms created in Dashpivot.

  3. Update the workspace brand colour (header divider line).

    1. Go to Home Folder > Settings > Branding.

    2. Adjust the brand colour used for the line separating the header from the form body.

  4. Confirm which metadata appears in the header (fixed for traceability).
The right side of the header includes metadata used for document control. This metadata cannot be removed.

  5. Modify folder names used in the header (Project and Team).

    1. In the left sidebar, locate the Project and Team folders where the form was created.

    2. Rename the folders as needed.
This updates the Project and Team values shown in the header.

  6. Modify the Template ID shown in the header.

    1. Open the template used to create the form.

    2. Click Edit Template.

    3. Go to the Settings tab.

    4. Update the Template ID.

  7. Understand which header versions cannot be changed.

    • Template Version: Automatically increases each time the template is edited and saved.

    • Form Version: Automatically increases each time the form is changed and saved.

  8. Understand which export metadata cannot be changed.
These values appear in PDF exports and cannot be modified:

    • PDF Creation Date and Time: Updates each time a PDF export is triggered.

    • PDF Page Number: Based on the number of pages in the form.

  9. Review other header fields and what can be modified.

    • PDF Export Disclaimer: Fixed text that says “Printed version is uncontrolled”.

    • Prefilled Text: Can be modified, up to 24 characters (including spaces and symbols).

    • Prefilled Image: Available for the header only. Can be modified by uploading an image.

Rules

  • Header metadata used for document control cannot be removed.

  • Template Version, Form Version, PDF Creation Date and Time, and PDF Page Number cannot be modified.

  • PDF Export Disclaimer cannot be modified and shows: “Printed version is uncontrolled”.

  • Prefilled Text is limited to 24 characters (including spaces and symbols).

  • Prefilled Image can only be used in the header and requires an uploaded image.
